Objective: To understand the enrollment status of residents in County A, Shanxi Province, explore its influencing factors, and provide countermeasures and suggestions for relevant departments to improve the residents' enrollment rate. Methods: We gained an overview of the development of medical insurance in China through literature research. From October to December 2023, a multi-stage stratified random sampling method was employed to conduct a questionnaire survey on the enrollment situation of residents in County A, Shanxi Province. A total of 355 valid questionnaires were collected, and the results were analyzed using SPSS software. Results: The correlation analysis results indicated that education level, health status, annual household income, level of understanding, satisfaction with the service quality of medical staff, and satisfaction with the overall service quality of medical institutions influenced residents' enrollment behavior (P < 0.05). Logistic regression analysis revealed that annual household income and satisfaction with the service quality of medical staff had significant impacts on whether residents enrolled in medical insurance (P < 0.05). Conclusion: The enrollment rate of residents in this county was 95.83%, which was relatively high, but there still existed a non-enrolled population. To address the issue of non-enrollment, it is recommended to improve the enrollment rate by broadening the channels for medical insurance publicity, increasing the reimbursement ratio of medical insurance, strengthening the construction of grassroots medical talent teams, and utilizing big data to enhance the supervision and management of medical insurance funds and medical institutions. Additionally, implementing various preferential policies should be carried out to effectively help residents reduce their medical burdens and enhance public trust by making medical insurance expense trends transparent.
