Abstract: The research paper has been published to develop a brief idea regarding the implementation of TQM&5s and its result on the hospitals performance including patient satisfaction. For the betterment of this study four Leading Tertiary Care hospitals have been considered. Again, the model of SERVQUAL is taken to assess the clinical quality and Patient satisfaction as well. In this project, the role of the SERVQUAL model is well discussed which gives a clear idea on measuring customer satisfaction. As a result of this study, it has been found that quality needs to be measured at several stages such as unit level, organization level, and individual level. Though the positive impact on the implementation of the SERVQUAL model has been discussed in this paper, there are also some barriers that need to be solved by the hospitals to properly implement TQM&5s.

Abstract: Introduction: Postoperative sore throat (POST) and hoarseness is an on-going worldwide challenge after general anaesthesia. In this study, we hypothesised that patients intubated with the fiberoptic bronchoscope–guided intubation device differs in POST and hoarseness from other patients intubated with the oral Macintosh laryngoscope–guided intubation device. If this could be confirmed, the guideline of peri anaesthesia may need to be different. Objective: This study aimed to investigate whether fiberoptic intubation can provide increased patient safety and comfort compared to standard laryngoscope intubation performed by nurse anaesthetists. Methods: Within this single-blinded, consecutively controlled clinical trial two hundred and nine patients scheduled for elective neurosurgical treatment requiring intratracheal intubation, were randomly divided into intervention and control group. The intervention group received fiberoptic bronchoscopy–guided intubation during anaesthesia as well as nursing and postoperative treatment based on conventional drug therapy after neurosurgical treatment. The control group received intubation during anaesthesia, as usual, using Macintosh laryngoscope intubation, and postoperative treatment based on conventional drug therapy after neurosurgical treatment. Data regarding the patient safety were obtained during peri anaesthesia, and data concerning the patient comfort were collected on the first postoperative day. Statistical analyses were performed using SPSS version 22. Results: Seven patients were excluded (3.3%); thus, the final population consisted of two hundred and two patients: 62.4% women, the mean age of 52 years (20 to 86 years), 106 (52.5%) patients in the fiberoptic intubation group. A statistically significant difference was found between groups according to patient safety, measured by the number of anaesthetists involved P=0.024 in favour of the fiberoptic intubation device. A trend was pointing to increased pain in the mouth and throat P=0.053, when intubated with the laryngoscope. Other patient safety and comfort measurements showed no statistically significant differences between groups. Conclusions: The study directs attention to some possible benefits of using the fiberoptic intubation device, which may improve patient safety and comfort when compared to standard laryngoscope intubation.

Expression of Fhit, hWAPL and Survivin in Cervical Squamous Cell Carcinoma and Significance Thereof

Abstract: Objective To preliminarily discuss the role of fragile histidine triad (FHIT) gene, human wings apart-like (hWAPL) gene, survivin gene in lymphatic metastasis of cervical squamous cell carcinoma. Method Expression of FHIT, hWAPL and survivin in 30 cases of normal squamous epithelium, 30 cases of CIN III and 60 cases of cervical squamous cell carcinoma was detected using the immunohistochemical technique. A statistical analysis was conducted for the relationship between the expression levels of FHIT, hWAPL and survivin and clinical pathologic parameters through expression count LVD of FHIT. Results FHIT value is 1.93±0.57 in the group of normal squamous epithelium, 2.87±0.63 in the group of CIN III and 4.12±0.72 in the group of cervical squamous cell carcinoma. The difference has statistical significance (P<0.05). The positive expression of hWAPL in normal cervical tissues is 0.00% (0/30), the positive expression rate is 36.67% (11/30) in the group of CIN III, and the positive expression rate is 63.33% (38/60) in the group of cervical squamous cell carcinoma. The difference has statistical significance (P<0.01). The positive expression rate of survivin in normal cervical tissues is 0.00% (0/30), the positive expression rate is 30.00% (9/30) in the group of CIN III, the expression rate is 61.67% (37/60) in the group of cervical squamous cell carcinoma. The difference has statistical significance (P<0.01). Conclusion FHIT, hWAPL and survivin promote lymphangiogenesis during occurrence and progression of cervical squamous cell carcinoma and lymphangiogenesis plays a vital role in lymphatic metastasis of cervical squamous cell carcinoma.

Abstract: This evaluation draws evidence on the effectiveness and outcome of learning and teaching courses that were offered to a number of teaching staff of the Academies of Health Sciences, Midwifery Schools and Centres for Continuous Professional Development in Sudan. It is based on a cross-sectional study consisting of self-administered questionnaires conducted from October 2017 to January 2018. The questionnaires were used to measure the extent to which the participants of the training courses acquired knowledge and skills and applied these skills in their jobs. Data was analysed using Excel sheets and SPSS version 21 and significant tests performed. The response rate for the self-administered questionnaire was 80%. Most of participants were females, early middle-aged, bachelor’s degree holders with more than 9 years of experience in the area of teaching health care professionals. The results are presented following the Kirkpatrick framework for evaluation of training, which was used to measure participants' reactions, learning and change of behaviour. Most of the participants were satisfied with the design and content of the course but expressed concerns regarding its duration and lack of residential facilities for course participants. Pre and post tests were used to measure learning. Comparison of the two results showed that participants gained knowledge in writing learning outcomes, although presentation skills showed no improvement following the training and the use of technology remained a challenge even after the course. In terms of application of the newly gained knowledge and skills, the findings illustrated that over 90% of the participants were satisfied with what they had learned after 3 months or more following the training course, and over 65% of the participants stated that the knowledge gained from the training remained useful. In conclusion, short courses on teaching and learning can be a valuable investment to both instructors and students. The evaluation has shown that course participants were satisfied with the training courses, gained new knowledge, and were able to apply the acquired knowledge and skills without any difficulties. It is therefore suggested to cascade these training courses to all remaining States in Sudan in order to improve the skills and capabilities of teaching staff.

Abstract: Background: Acute malnutrition is one type of under-nutrition that is devastating life and as reflected a nutritionally deficient state of recent onset related to sudden food deprivation or mal-absorption utilization of nutrients which results significant weight loss. In Ethiopia, child under-nutrition is one of the most serious public health problem and the highest in the world. Therefore, this study designed to assess the prevalence of acute malnutrition and its associated factors in children aged 6-59 months in Gursum district in Somali region in Ethiopia. Method: A community-based cross-sectional study was conducted on 541 children aged 6–59 months from Feb. 14–25, 2019. A multistage sampling method was used to select the study participants. Interviewer administered questionnaire was used and EPI-DATA statistical software were entered the data and SPSS version20 were used for analysis. The Z-scores for weight-for-height (WHZ) were calculated using the World Health Organization anthro2010. Binary logistic regression was used to identify the factors associated with acute malnutrition. The statistical significance Variable was declared at P-Value less than0.05. Multi-co-linearity effect was checked. The study found the prevalence of wasting was 21.2% (17.54, 24.726). Educational status of the mother (AOR=1.87, 95% CL: 1.065, 3.286), diarrhoea two weeks before interview (AOR=2.469, 95% CI: 1.487, 4.100), family size (AOR=1.72, 95% CL: 1.058, 2.797), colostrum feeding (AOR: 2.26; 95%CI: 1.404, 3.646), bottle feeding (AOR: 3.297; 95% CI (1.944, 5.591) were significantly associated with acute malnutrition in Gursum district. Conclusion: The prevalence of acute malnutrition in the study area was high. Health extension workers and stakeholders should give concern on promotion of proper nutrition in the community.
